Overview

The Community Health Worker will be a key community facing team member working in the community on behalf of the Diabetes Research Education and Management Team (DREAM Team). This position will assist in building and fostering community partnerships, implementing community facing programs and assisting community residents in their health and education needs. This will include activities related to the Mobile Diabetes Education Center (MOBEC), Diabetes Prevention Program (DPP), Success with Diabetes Program and Pharm2Home Initiative as well as any grant related initiative.

Responsibilities

General Ongoing Administrative Tasks:

  1. Record keeping of community member and program interactions.
  2. Performs general administrative tasks including but not limited to written and verbal correspondence.
  3. Follows and implements policies and procedures as established by the University and the Diabetes Institute.
  4. Assists in any grant or program required reporting.
  5. Maintains strict confidentiality regarding interactions with clients/patients.
  6. Attends any relevant DREAM Team/Diabetes Institute meetings.

 

Community Health Worker Role:

Assists in grant-funded activities, including:

  1. Conducting assessments in the community on CHWs employment in health plans, community hypertension awareness projects, healthy kids programs, etc.
  2. Holding training workshops for community members
  3. Working with local healthcare facilities and community-based organizations to implement chronic disease management services

SPECIFIC RESPONSIBILITIES: are those work assignments which are predominant, regular and recurring.

  1. Participates as a Community Health Worker in grant-funded activities which may include, but not limited to, establishing working relationships with community members, providing educational services to program participants.

 

Qualifications

Required

  • High school graduation or GE certificate.
  • Completion of the State of California approved Community Health Worker Certificate
  • Computer skills, MS Office 365 (Word, Excel, Power Point, Outlook), Outlook & Google Calendar
  • Internet Searching Abilities including Google Calendar

 

Desired:

 

·       Prior experience in a community-based or health-care setting related to providing chronic disease management services.

·       Experience working with under-resourced community members or groups.

Maximum Salary

USD $25,262.00/Yr.

Minimum Salary

USD $20,209.60/Yr.
